As an LPN within our facility, you will provide comprehensive geriatric nursing care and ensure a safe, therapeutic, and hygienic environment for our residents. You will work under the supervision of an RN or physician, including medication administration, treatment procedures, and supervision of subordinate staff to ensure the highest level of resident care.

Your Impact:

  • Specialized Nursing Care: Administer medications and perform treatments, including wound care, tracheostomy care, and catheter care, within the scope of your license.
  • Resident Monitoring: Observe and report on the effectiveness of medications and treatments, ensuring residents' needs are met through interdisciplinary team collaboration.
  • Care Planning: Participate in developing Minimum Data Set (MDS) assessments, which are comprehensive assessments of a resident's functional capabilities and health status, quarterly reviews, and care plans to ensure personalized and effective care for each resident.
  • Leadership and Training: Act as a lead or supervisor, overseeing and training subordinate staff in resident care practices. Participate in staff development programs and in-service training to enhance team skills and knowledge.
  • Communication: Ensure clear and effective communication with residents, families, and staff to facilitate optimal care and support.

Job Duties:

As an LPN you will:

  • Administer medication and performs treatments on an assigned unit as ordered by the physician and within scope of license.
  • Medications may be in a variety of forms including oral, enteral, parenteral, suppository, etc. Treatments may include sterile and aseptic wound cleaning and dressing, postural drainage and percussion, tracheostomy, gastrostomy and jejunostomy care and catheter care.
  • Observe and report the effectiveness of medications and treatments.
  • Ensure an adequate amount of supplies and medication are available.
  • Receive and deliver a variety of information regarding the residents’ medical, social, functional and other needs.
  • Participate in the receiving and giving of shift reports.
  • Communicate with residents, family and staff as necessary to ensure the resident receives appropriate care from the interdisciplinary team.
  • Complete the initial admission nursing assessment tool on a newly admitted resident.
  • Supervise subordinate staff
  • Assist as need with direct resident care for the completion of Activities of Daily Living (ADL’s) including bathing, dressing, observations, treatment and summary of resident progress.
  • Conduct and participates in in-service training and staff development programs for regularly employed staff and other duties as assigned.
  • Attend and participates in staff meetings.
  • May collect and analyze data for quality improvement and infection control programs.
  • Train new employees.
  • Performs other technical duties as assigned (e.g., collects specimens, responds to emergency situations rapidly and appropriately)
Knowledge, Skills & Abilities (KSAs):

Knowledge of:

  • Gerontological/geriatric nursing theory and practice
  • Resident condition and changing medical/physical /psychological care needs
  • Effects of the aging process
  • Practices and procedures of team nursing
  • Practices and procedures for implementing treatment plans for resident/patient care
  • Practices and procedures of pharmacology and medicines
  • Federal, State and agency laws, rules and regulations governing general practical nursing practices and the security and control of medical supplies and medications and dispensing thereof.

Skill in:

  • Practical nursing methods and techniques
  • Observing and reporting behavior and progress of residents/patients
  • Applying and interpreting State and agency laws and regulations governing geriatric nursing practices
  • Assisting in the development and implementation of patient care plans within a team nursing concept
  • Oral and written communication 

Ability to:

  • Assess client needs and evaluate usefulness or suitability of resources
  • Work with difficult people; establish and maintain excellent working relationships with clients, client families, team member and service providers.
Selective Preference(s):

One or more years of Skilled Nursing Facility or Long-Term Care Experience preferred.

Pre-Employment Requirements:
  • Current Negative TB skin test (Mantoux skin test) within the last twelve months or a written statement from a physician, physician’s assistant or a registered nurse practitioner indicating freedom from Tuberculosis, if in the past has had a positive skin test or Tuberculosis
  • Must be able to obtain and retain a fingerprint clearance card issued by the Arizona Department of Public Safety
  • Current or temporary license to practice as a LPN nurse in the State of Arizona
  • Current CPR card that was obtained by physical assessment
