The Arizona Pioneers’ Home is a continuing care retirement home operated and funded by the State of Arizona.
The Home provides care and services to residents living independently, as well as, to those requiring personal, assisted living, intermediate and skilled care.
• Administering medication treatment in a safe, timely manner in accordance to all applicable rule, regulations, laws, policy and procedures
• Administration of treatments and documentation in a safe, timely manner in accordance with all applicable rules, regulations, laws, policy and procedure
• Documenting and communicating residents care and condition both written and verbal to interdisciplinary team and other health care settings to ensure continuity of care with adherence to HIPPA regulations
• Performing assessments and preparing incident reports, infection control reports and participates in QAPI projects to improve care as requested
• Supervising nursing assistants assigned on a shift of care to ensure quality care and adherence to AZPH policies and procedures is provided
• Resolving interpersonal conflicts, work procedures issues, and answers questions regarding care; Assisting in training and reinforcement of care needed during a shift of care
• Function as an advocate for residents in the delivery of quality of care with regard for resident’s rights, wishes and directives
• Present a professional demeanor an interact with residents, co-workers, resident’s families, visitors, volunteers and vendors while maintaining confidentiality of protected health information
• Performing assessment and documenting finding to physician as indicated
• Initiating, reviewing, and updating resident care plants
• Documenting care provided in medical records
• Other duties as assigned
Knowledge of:
• Standards of Geriatric Nursing Practice, care needs of the geriatric population, team nursing, effects of aging process on ADL management and medical care needs
• Federal, State and Agency laws governing general nursing practice
Ability to:
• Perform quality nursing assessments, perform basic nursing procedures to include but not limited to wound care management, Foley Catheter management, pain assessment and interventions, medication management and pharmacological impact on the resident
• Communicate both verbally and in writing to members of the team including the medical provider
• Effectively perform assessments that result in proper interventions
• Perform basic nursing care within the scope of practice
• Work effectively on an interdisciplinary team
• Must have patience, tact and a maturity level with which to interact with people at various levels mental functioning
• Assure that all residents are treated fairly with kindness, dignity and respect
• Possess a valid fingerprint clearance card or an application to be submitted within twenty days of hire to the Department of Public Safety
• Possesses a license to practice as a Licensed Practical Nurse in the State of Arizona
• Possess a current CPR Card and First Aid
