The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a Nurse Support Coordinator Liaison. This position is a registered nurse responsible for key communication, reporting, interdepartmental updates, interdisciplinary meeting attendance, and clinical escalation between Support Coordination and Healthcare Services. This position will report to the Health Care Services Medical Director. This position with help to ensure that members of the Division of Developmental Disabilities have effective care coordination for their clinical and L TSS needs.

Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation, cannot have a principal interest in or have a spouse or close family with principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor.

This position may offer the ability to work remotely, within Arizona, based upon the department's business needs and continual meeting of expected performance measures.

Job Duties:

Essential Duties and Responsibilities include but are not limited to:

  • This position participates as the nurse liaison between Health Care Services and Support Coordination for the Nursing families, Support internal and Coordinator external (NSC) unit; attends department meetings, and managers, collaborates and communicates with Division members/ related to the physical, mental and psychological needs of Division members27
  • Provides reliable communication between multiple functional areas including bidirectional communication between Support Coordination and Health Care Services. Coordinates and encourages interdisciplinary collaborations to address issues/concerns affecting Division member's needs according to the member's Person-Centered Service Plan (PCSP) and Nursing Assessment.
  • Reports and escalates issues/concerns relating to member's skilled nursing care, medications, immediate discharge needs, and services to appropriate department management and Directors. Helps to recognize and problem-solve when social determinants of health are involved.
    Attends multidisciplinary meetings, including but not limited to special team meetings, discharge planning, IDT, and OIDT meetings. Helps to ensure the medical director or CMO are involved when indicated.
  • Provides Nurse Support Coordination Program updates to Health Care Services. Provides Health Care Services updates to the Nurse Support Coordination Program. Participates in policy writing when related to Nursing Support Coordination and Health Care Services. Facilitates compliance with NCQA, AHCCCS and DOD medical management standards by sharing information with the Nurse Support Coordinator unit.
Knowledge, Skills & Abilities (KSAs):

Knowledge of:

  • AHCCCS, Division, State and Federal Policies.
  • Changing medical; physical; behavioral; nutritional; and social care needs of people with intellectual/ developmental disabilities (I/DD).
  • Care management concepts, principles, and strategies.
  • Nursing theories and practices related to individuals with intellectual and/or developmental disabilities and behavioral health conditions.
  • Nurse Practice Act, person-centered planning strategies.

Skilled in:

  • Using computer software (e.g., Google Suites including Gmail, Google Docs, Google Sheets, Google Slides, Google Forms; Microsoft Word, Excel, Adobe Acrobat, and other databases, etc.).
  • Excellent oral and written communication.
  • Comprehensive nursing assessment & needs analysis.
  • Analytical and clinical skills.
  • Developing, analyzing and implementing health treatment plans.

Ability to:

  • Work independently.
  • Think critically and problem-solve.
  • Observe and assess the health status and needs of individuals with intellectual and/or developmental disabilities and behavioral health conditions.
  • Interpret and apply DOD Policies and Procedures.
Selective Preference(s):

The ideal/preferred candidate will possess:

  • Two years experience working as a registered nurse with individuals with disabilities, in-home health, hospice, or long-term care, and knowledge of the current Medicaid service delivery system.
  • A Bachelor of Science in Nursing (BSN) from an accredited college or university is preferred.
Pre-Employment Requirements:
  • Required at time of appointment: possession of a current license to practice as a registered nurse in the State of Arizona.
  • In-state travel is required, driving or the use of a vehicle as an essential function of the job to conduct State business, then the following requirements apply: Driver’s License Requirements. Any employee operating a motor vehicle on State business must possess and retain a current, valid class-appropriate driver's license, complete all required training, and successfully pass all necessary driver's license record checks. The license must be current, unexpired, and neither revoked nor suspended. Employees who drive on state business are subject to Driver's license record checks, must maintain acceptable driving records, and must complete all required driver training. An employee who operated a personally owned vehicle on state business shall maintain the statutorily required liability insurance (see Arizona Administrative Code (A.A.C.) (R2-10-201.11).
  • Candidates for this position shall be subject to a search of both the Department of Child Safety Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459.
  • Successfully pass background and reference check; employment is contingent upon completion of the above-mentioned process and the Agency’s ability to reasonably accommodate any restrictions.
  • All newly hired State employees are subject to and must successfully complete the Electronic Employment Eligibility Verification Program (E-Verify).
  • Employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41-1968 in order to work with children and vulnerable adults.
